Healthcare services are available only where the treating clinician:
Holds an active and appropriate professional license
Is authorized to provide the particular service
Can comply with applicable state law
Has determined that telehealth or in-person treatment is clinically appropriate
The location of the patient at the time of care may determine which state’s licensing and healthcare laws apply.
You agree to accurately disclose your physical location during telehealth encounters.
The Website’s availability in a state does not mean every service advertised is available in that state.

22. Insurance and Reimbursement
Unless expressly stated otherwise, Longevity Concierge Med may operate as a direct-pay or cash-pay practice and may not participate with health insurance plans.
We do not guarantee that:

A service is covered
A laboratory or pharmacy is in network
A submitted superbill will be accepted
A payment will count toward a deductible
You are responsible for confirming benefits and coverage directly with your health plan.
23. Privacy
Use of personal information collected through the public Website is governed by our Website Privacy Policy.
Use and disclosure of patient PHI are governed by our HIPAA Notice of Privacy Practices when HIPAA applies.
The two documents address different categories and contexts of information.
Information submitted to a general public website form may not automatically become part of a medical record or receive the same treatment as information submitted through an approved clinical system.

The general Website is intended for adults.

Your continued use of the public Website after revised Terms are posted constitutes acceptance of the revised Terms, except where additional affirmative consent is legally required.
Changes to a signed membership, financial, or patient agreement will be handled under that agreement and will not occur merely through an update to these Website Terms.
34. Severability
If a provision of these Terms is found invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law.
The invalid provision will be interpreted or limited as closely as legally possible to its intended purpose.
35. No Waiver
